Before coming to see our Consultant Physicians, ask yourself what specifically are the problems you are facing in your life since your cancer diagnosis and treatment. A reliable checklist you can refer to is called the “Distress Thermometer and Problem list”, which can be downloaded here: Checklist It takes just 5 minutes to complete and can help prompt the conversation with you, your family and carers as to what current ongoing or future problems you face.
Medicare may help to pay for your Survivorship Plan and other services provided by Evexia Care, but it is essential that you obtain a medical referral from your general practitioner and/or oncologist before your appointment.
Extended consultations are available for an additional fee.
If you are travelling from a rural area you will need to check whether you can claim Isolated Patients Travel and Accommodation Assistance (IPTAS) given this is a specialist service that may not be provided in your area.

Health Funds will not fund your visit to Evexia Care as this is an outpatient service.
Onwards referrals recommended to improve your wellbeing in your Survivorship Care Plan such as to exercise physiology, physiotherapy, lymphoedema, dietetics / nutrition may be covered by a chronic health care plan your General Practitioner can formulate. In addition some health funds may cover this as well as oncology massage, acupuncture and other integrative therapies such as yoga that have demonstrated benefits in wellness and recovery after a cancer diagnosis.
