Patients need a Referral from their Medical Practitioner

Patients require a referral from either their General Practitioner (an indefinite referral is preferred) or Specialist to see one of our physicians.

It is known that carers and partners of patients with cancer also experience a high level of distress and effects on their health. If this is the case we ask referrers to suggest that family members and carers attend the appointment as well.

Referral information should include:

• Your Name
• Contact details and Provider number
• Diagnosis and reason for referral
• Patient details:
  • Full name
  • Date of Birth
  • Contact details including address
  • Telephone number (home and daytime)
  • Medicare details
  • Language spoken if not English, in which case the patient will have to bring an interpreter
